Tribute Giving
Memorial Giving – remembering our loved ones 
Family members or friends can easily and thoughtfully be remembered with a donation to Milton District Hospital Foundation. A memorial gift will benefit Milton District Hospital Foundation and celebrate the life of your loved one.
When memorial donations are received, a notification will be mailed to families to acknowledge these gifts. Cumulative donations of $1,000 or more will be honoured with a listing of your loved one’s name on our Memorial Wall located in the main lobby.
To make a memorial donation you have several options:
- To donate on-line please click the Donate Now button on this page. In the message/instruction box please provide the name of the person in whose memory you are donating and the name and address of the next of kin for acknowledgement purposes.
- To mail your donation please print the Donation Form and mail to:

In Honour Giving - honouring those we cherish
Celebrate special events like Mother’s Day, weddings, anniversaries, gradations, holidays or to honour accomplishments with a tribute gift to Milton District Hospital Foundation.
Commemorate any occasion with a donation and the Foundation will notify the recipient of your gift with a card. Several options are available:
